Form Creation - Create a resource that can be emailed or texted to an individual - No PDF Mapping

This tutorial will show you how to create a resource that can be emailed or texted to an individual.  This tutorial does not include PDF mapping.  If you need to be able to fill in some spaces and then have it sent to an individual please see the PDF Mapping tutorial.

PART 1 - UPLOAD THE RESOURCE TO APEXFORMS
- Log in to Apex Forms
- Click "Media" on the left hand side
- Select "Add New" and drag/drop or click "Select Files" to upload a PDF
* Please note - for this type of resource, PDF's work best

PART 2 - CREATE A FORM
- Click "Forms" on the left hand side
- Click "Add New" from either the left hand side or the button towards the top of the page
- It will prompt you to add a title and a description.  Once you've filled this out click the "Add New" button.

PART 3 - ADD A FIELD TO CHOOSE TO TEXT OR EMAIL THE MATERIAL
- Select the "Checkboxes" option from the right side of the page.  It will add a checkbox field to the middle of the page.
- Click on the box that appeared. It will open up underneath with options.
- For the field label enter something like "Send To"
- As the "Choices" enter "Email" in one box and "SMS/Text" in the other

PART 4 - ADD FIELD TO ENTER EMAIL INFO
- From the right side under "Advanced Fields" select "Email".  Click on the box and select the "Advanced" tab
- Select the box that says "Allow Conditional Logic"
- The text that pops up should read "Show this field if ALL of the following match: 
Send to: is Email

PART 5 - ADD FIELD TO ENTER SMS/TEXT INFO
- From the right side under "Advanced Fields" select "Phone". Click on the box and select the "Advanced" tab
- Select the box that says "Allow Conditional Logic"
- The text that pops up should read "Show this field if ALL of the following match:
Send to: is Text

PART 6 - SET UP EMAIL PDF
- Go to "Settings" under the form title
- Select "Notifications" on left
- Select "Add New"
- Name your Notification
- For "Send To" select "select a field"
- For "Send to field" select "Email"
- Add a subject for the email
- Under "Message" select "Add Media".  Select the PDF you uploaded earlier
- Select "Enable Conditional Logic"
- The text that pops up should read "Show this field if ALL of the following match:
Send to: is Email
- Select "Update Notification"

PART 7 - SET UP TEXT PDF
- Go to "Settings" under the form title
- Select "Twilio" on the left
- Select "Add New"
- Enter a Name for the Text
- Select your Twilio number from the drop down
- For "Send To" select "phone"
- For the message enter the URL of the PDF.  (This can be found by selecting the "Media" tab, clicking on the PDF, then clicking on the "Copy Link" link on the right side)
- Next to "Conditional Logic" select "enable"
- The text that pops up should read "Show this field if ALL of the following match:
Send to: is Text
- Click "Update Settings"

PART 8 - PUT YOUR FORM ON A PAGE
You'll need to put your form on a page in order to share it's URL.
- Click "Pages" on the left hand side
- Click the "Add New" button
- Add a page title
- Click the + sign on the top left
- Select "Embeds"
- Select "Forms"
- From the drop down select the form that you just created
- Select "Publish" in the top right.  You will have to confirm this and may be prompted to select "Publish" a second time.
- A page URL will pop up on the right hand side.  Copy it.

PART 9 - ADD THE FORM TO YOUR APP
- Go back to the CMS
- Select "Add New"
- Add the Title/Description
- For the "Type" select "Link". Paste the form link into the URL space
- Click Save Changes
